Improve community health by individual behaviour change through the development, implementation, and monitoring of policies related to the four priority areas that support and promote healthy living in Windsor-Essex County. To encourage relevant health service providers to develop, adopt, and implement health policies through the use of the 4 Small Changes initiative (consistent messages strategy). Develop 4 Small Changes resource policy package; this package will include:
- Suggested activities and tools related to the four priority areas in the 4 Small Changes initiative.
- Sample policies and supports related to the four priority areas in the 4 Small Changes initiative.
- Create a common evaluation tool and protocol to measure the effectiveness of 4 Small Changes initiative in support of health policy development.
Re-evaluate and develop the 4 Small Changes website to include more relative links, different age groups, and technical functionality.
Include 4 Small Changes initiative on the Go For Health Website to promote and disseminate 4 Small Changes resources and policy supports.
Develop 4 Small Changes policy promotional materials.

Development and distribution of resources supporting policy strategies related to the 4 Small Changes consistent messaging initiative.

Common evaluation tool developed and implemented.

Improve community health by conducting a health sector policy scan to create a baseline/benchmark for policy strategies. To complete a health sector policy scan for benchmark, planning, and progress tracking purposes. Identify health service providers to complete survey.
Develop policy scan online survey.
Analyze data and develop final report with key findings, gaps and recommendations.
 

Completed health sector policy scan with report.
 

Report will be shared with health sector agencies for reference and recommended policy actions.
